Job summary

Simply Helping is seeking a Care Coordinator to join our Torquay team. You will coordinate safe, responsive, and person-centred services that meet each client’s needs, acting as the main point of contact for clients, families, support workers and other stakeholders to ensure high-quality service delivery.

You will manage care plans, onboarding, and service delivery while maintaining accurate records and supporting recruitment and training of staff.

Qualifications

• Diploma in Community Services with 3+ years' case management experience or
• Bachelor’s degree in Social Work, Behavioural Science or related discipline; or
• Nursing qualifications with relevant industry experience.

Responsibilities

• Coordinate and review person-centred care plans
• Schedule and monitor client services and support
• Liaise with clients, families, support workers and external stakeholders
• Support client onboarding and service commencement
• Monitor service quality and identify opportunities for improvement
• Assist with recruitment, onboarding and training of support workers
• Maintain accurate client records and documentation
• Support operational and administrative functions as required
• Escalate service risks and concerns to management
