Job summary

Pearl Home Care Pty is seeking a dedicated Care Co-ordinator to manage an assigned group of clients from onboarding through ongoing review. You will tailor plans to individual goals, maintain regular contact with families, and collaborate with clinical teams to ensure safe, independent home care.

Key duties include care planning, budget understanding, coordinating services, and staying compliant with Aged Care Quality Standards. A flexible hybrid role with support from a local team awaits.

Qualifications

• Experience in care coordination, case management, nursing, allied health, home care or aged care.
• Strong care planning, organisation and time management skills.
• Working understanding of Support at Home, Aged Care Quality Standards and client rights under the new Aged Care Act.
• Good judgement in recognising risk and complex needs, and when to upscale.
• Clear, warm communication with clients, families, support workers and health professionals.
• Preference for RN/EN or allied health qualification; minimum: Certificate III in Individual Support.
• Current First Aid and CPR; valid driver's licence and insured vehicle.

Responsibilities

• Welcoming new clients and building care plans around goals, preferences and wellbeing.
• Reviewing care plans regularly and when needs change.
• Keeping in regular contact with clients and families and being their main contact.
• Helping clients understand their Support at Home funding, budgets, statements and choices.
• Coordinating services with scheduling team, support workers and external providers.
• Arranging assistive technology, home modifications, allied health, and restorative and end-of-life supports.
• Spotting changes in health, risk or circumstances and escalating clinical concerns.
