The role involves travel within the local area to visit clients, ensuring compliant, person-centred care with ongoing improvements and effective communication with clients and families.

Qualifications

• Experience with care planning and budgeting for client packages.
• Strong written and oral communication skills.
• Ability to work autonomously and in a team.
• Current Working with Children Check (WWCC) or equivalent.

Responsibilities

• Create and maintain client files and documentation, ensuring accuracy.
• Update files with reports, care plans and service agreements.
• Administer budgets and ensure services fit funding.
• Liaise with stakeholders to communicate effectively and build rapport.
