Job summary

Curamoir Healthcare Recruitment – Gregory Hills NSW is seeking a dedicated care coordination professional to manage an allocated caseload of NDIS participants. You will ensure care plans, risk documentation and service delivery stay current and aligned to individual goals, while collaborating with intake, rostering and compliance teams.

You will provide day-to-day leadership to support workers and staff, coach and supervise training, and escalate high-risk matters through internal channels to

Qualifications

• Experience in care coordination/case management within disability or community services.
• Knowledge of NDIS processes, documentation and safeguarding requirements.
• Strong communication, organisational and CRM/MS Office skills.
• Current driver licence and ability to do home visits as required.
• Clear checks: NDIS Worker Screening, WWCC, police check.

The role:

You will coordinate safe, responsive supports for an allocated caseload of NDIS participants, ensuring care plans, risk documentation and service delivery remain current, accurate and aligned to individual goals.

You will work closely with intake, rostering and practice/compliance stakeholders to support smooth onboarding, continuity of care, and timely responses to changes in needs. You will also provide day-to-day leadership to allocated support workers and business support staff, including coaching, supervision and follow-up on training and competency requirements, while escalating incidents, safeguarding concerns, restrictive-practice issues and other high-risk matters through the appropriate internal pathways.

Requirements:

Qualification in disability, allied health, social science, welfare, community services (or equivalent relevant experience)

Experience in care coordination/case management within disability or community services

Working knowledge of the NDIS, documentation, incidents and safeguarding

Strong communication, organisation and systems capability (CRM/MS Office)

Current driver licence; able to do home visits as required

Current checks as required (NDIS Worker Screening, WWCC, police check)
