NDiCare is seeking a motivated professional to join our team as a Disability Support role focused on coordinating care for NDIS participants in Melbourne. You will manage schedules, roster replacements, and support a team of Disability Support Workers while contributing to care planning and client assessments.
The position involves maintaining documentation, liaising with participants and stakeholders, and participating in after-hours/on-call duties with relevant rates.

NDIS Workers Screening Clearance (Or willing to obtain).
Working with Children Check (Or willing to obtain).
Criminal Record Check (Or willing to obtain).
Experience in Case Management and/or relevant Case Management Qualifications (Desired but not required).
Preparing and transmitting schedules for customer services.
Rostering - Arranging replacement staff to cover planned or unplanned absences and shift changes.
Stake Holder Management - Take ownership for developing and retaining/maintain these networks.
Taking of enquires in relation to current and potential NDIS participants.
Assist and support a team of Disability Support Workers.
Contribute to the development, implementation, monitoring and review of care plans to meet clients care needs and to promote quality of life.
Promote a consultative and collaborative approach to client care planning and quality managed activities.
Carry out comprehensive and accurate client assessments.
Maintain appropriate records and documentation. Complete all necessary administration to the required standard, ensuring full compliance
Fulfil other duties as required by management and other department personnel as requested/required. After-hours and on-call duties will be required, the appropriate award and standby rates will apply.
Occasional client transport.
