Job summary

Anglicare Southern Queensland at Hervey Bay is seeking a full-time Care Coordinator to lead clinical oversight of resident care and coordinate multidisciplinary teams. You will ensure compliance with statutory requirements and drive quality care across the facility, in partnership with the Facility Manager.

The role emphasizes clinical leadership, documentation for accreditation, and a focus on budget-conscious operations to maintain high standards of care for Queensland residents.

Qualifications

• Must have senior nursing experience in coordinating and evaluating aged care residents.
• Knowledge of accreditation standards and Aged Care Funding Tool and budget principles.
• Ability to lead and supervise a multi-disciplinary team and coordinate staff education.

Responsibilities

• Manage and monitor resident care and clinical systems (medication, continence, wound management, infection surveillance).
• Coordinate timely ACFI funding documentation and progress notes per procedures.
• Report on quality-of-care to Facility Manager promptly.
• Provide clinical leadership to nursing and care team.
• Coordinate care plan reviews, evaluations and care conferences.
• Assist Facility Manager with budgeted service delivery and resource management.
• Ensure documentation meets accreditation and funding body requirements.

Our Kirami facility is seeking a full-time, permanent Care Coordinator to join their team.

The purpose of the Care Coordinator is to ensure that all residents of our Facility receive high quality clinical care that meets their specific needs and complies with statutory and legal requirements. The Care Coordinator will provide clinical leadership in partnership with the Facility Manager and other care staff.

Key responsibilities

• Manage and monitor resident care and clinical systems, e.g. medication, continence, wound management and infection surveillance.
• Coordinate the timely completion of Aged Care Funding documentation assessment and progress documentation in accordance with facility procedures.
• Report on the quality-of-care services to the Facility Manager in a timely manner and in accordance with facility and Anglicare procedures.
• Provide clinical expertise and direction to the nursing and care team.
• Coordinate, monitor and maintain systems to ensure timely resident care plan reviews, evaluations and care conferencing.
• Assist the Facility Manager to ensure services and resources are managed within the approved budget.
• Assist in the management and monitoring of resident documentation to ensure it meets the requirements of accreditation and the funding bodies.

Key requirements

• Demonstrated experience in a senior nursing position in the coordination and clinical evaluation of aged care residents.
• Working knowledge of accreditation standards and Aged Care Funding Tool, budget principles and practice.
• Demonstrated ability to provide leadership and supervision to a multi-disciplinary team, identify professional development needs of staff and to coordinate education programs.

Mandatory requirements

• NDIS Worker Screening Clearance OR police check
• The ability to meet the requirements of a Key Personnel Check for the purposes of the Aged Care Act (2024)
• Degree in Nursing
• Post graduate qualification in a related field, e.g. Gerontology or Mental Health (preferred)
• Current registration with AHPRA
• Ability to work a 24/7 rotating roster, including weekends and public holidays
