Job summary

For Purpose Aged Care in Narangba, QLD is seeking an experienced Clinical Care Coordinator to lead our aged care residence. You will work closely with Clinical Managers and Residential Manager to develop and deliver exceptional resident care and ensure clinical standards are met.

You will mentor RNs, ENs and PCAs, oversee day-to-day clinical operations, and contribute to service delivery during times of change.

Qualifications

• 2+ years' experience as CCC or similar in aged care or acute sector.
• RN qualification with current AHPRA registration.
• IPC qualification advantageous or willingness to work towards.
• Police Check within 12 months.
• Excellent and up to date clinical knowledge and assessment skills.
• Experience in staff supervision and development.
• Knowledge of funding and accreditation processes.
• Knowledge of Care Planning.

We are currently seeking an experienced and qualified Clinical Care Coordinator to work closely with our Clinical Managers and Residential Manager to develop our stunning aged care residence based in Narangba.

The Clinical Care Coordinator (CCC) will provide day to day clinical leadership to the team, foster a culture of clinical excellence and oversee the clinical care of our residents to ensure that a high standard of care is maintained.

Skills and requirements for this role:

• Ideally at least 2+ years' experience in a Clinical Care Coordinator or similar role in the aged care or acute sector;
• RN qualification with current AHPRA registration;
• Australian Qualifications Framework Level 8 Infection Prevention Control (IPC) qualification advantageous or willingness to work towards;
• Police Check within 12 months;
• Excellent and up to date clinical knowledge and assessment skills;
• Experience in staff supervision and development;
• Strong communication and negotiation skills;
• A working knowledge of funding and accreditation processes;
• Knowledge of Care Planning.
