Catholic Archdiocese of Brisbane – Brisbane QLD is seeking a Cooperative Coordinator to provide leadership and operational oversight for aged care services. The role involves driving service excellence, ensuring compliance with Aged Care Quality Standards, and aligning with Centacare’s mission and values.

The position is full-time with a hybrid model and travel to Darra, Gold Coast, and Beaudesert. Remuneration includes $63ph + Super, Not-for-Profit salary packaging, and travel allowance.

Qualifications

• Qualified in nursing or community services with aged care or disability experience.
• Strong understanding of aged care funding models, legislation and compliance.
• Excellent communication, leadership and coordination across diverse service environments.
• Proven ability in care planning, reporting and problem-solving.

Responsibilities

• Lead regional service delivery planning with the Regional Manager.
• Oversee care planning, assessments, reviews and service delivery to meet quality standards.
• Supervise and coach Care Partners to build capability.
• Oversee Beaudesert Hub operations in collaboration with the Regional Manager.
• Monitor incidents, complaints, and feedback and drive timely improvements.
• Ensure services align with funding agreements, budgets and rostering.

About the role:

Centacare is looking for a Coordinator to provide leadership, coordination, and operational oversight to ensure the delivery of high-quality, person-centred aged care services. The Coordinator, plays a key role in driving service excellence, compliance with the Aged Care Quality Standards, and alignment with Centacare’s Mission, Vision, Values, and Catholic Social Teachings.

• Employment Status: Full Time
• Location: Hybrid model with WFH available with travel to Darra, Gold Coast, and Beaudesert
• Remuneration: $63ph + Super + Not for Profit Salary Packaging (Increases your take home pay) + Allowance Travel Reimbursement (KM's).

Key Responsibilities:

• Lead regional service delivery planning with the Regional Manager to ensure safe, person-centred, and compliant services.
• Oversee care planning, assessments, reviews, and service delivery to ensure quality standards and regulatory requirements are met.
• Supervise, coach, and support Care Partners to build capability and ensure best-practice service delivery.
• Oversee operations and facility coordination at Centacare’sBeaudesert Hub in collaboration with the Regional Manager.
• Monitor incidents, complaints, and feedback and drive timely improvements in service delivery in collaboration with the Regional Manager.
• Ensure services align with client needs, funding agreements, budgets, and rostering requirements.
• Monitor referral pathways and My Aged Care systems to support efficient client intake and transitions.
• Participate in a rotational after-hours on-call roster.
• Ensure compliance with WHS, safeguarding, privacy, and legislative requirements.

About You

• Qualified in Nursing, Community Services, Aged Care, Disability or related field (or possess significant demonstrated relevant industry experience).
• Strong understanding of Aged Care, CHSP and Support at Home funding models, legislation, and compliance requirements.
• Excellent communication, leadership, stakeholder engagement, and coordination skills across diverse service environments.
• Proven ability in care planning, reporting, problem-solving, conflict resolution, and working independently with sound judgement.
