Job summary

Aspen Medical invites Registered Nurses to join a community healthcare initiative across Central Coast and Central West NSW, delivering care in patients' homes with autonomous practice and clinical support.

Applicants should have unrestricted AHPRA RN registration, experience in community/aged care, strong assessment and care planning skills, and a valid Australian driver’s licence with willingness to travel regionally.

Qualifications

• Current, unrestricted AHPRA registration as a Registered Nurse.
• Experience in community health, home care, aged care or primary healthcare.
• Strong clinical assessment and care planning skills.
• Autonomous work ability with sound clinical judgement.
• Compassionate, patient-focused approach with good communication.
• Australian driver’s licence and willingness to travel regionally.

Responsibilities

• Deliver person-centred care to older Australians in their homes.
• Undertake comprehensive clinical assessments and individualized care plans.
• Provide wound management, medication administration and chronic disease care.
• Work independently across community visits and escalate concerns as needed.
• Build relationships with patients, families, GPs and allied health professionals.
• Maintain accurate documentation and support clinical safety and improvement.
