365 Care is seeking an experienced Registered Nurse to support older Australians in their own homes across Western Sydney. This highly autonomous role is primarily community-based, with travel across the Western Sydney region from our Penrith office and a preferred 0.6 FTE (3 days per week).

You will deliver nursing and personal care, conduct health assessments, and coordinate with families and the care team to keep clients safe and independent at home.

Competenze

• Experience in aged care, home care or community nursing.
• Current AHPRA registration as a Registered Nurse (Division 1).
• First Aid and CPR certificates.
• Driver licence and access to a registered and insured vehicle.
• NDIS Worker Screening Check
• Working With Children Check
• NDIS Worker Orientation Module

• Delivering nursing care, personal care and clinical support to aged care clients in their homes, ensuring they can continue living safely and independently at home.
• Conducting health assessments, reviews and ongoing monitoring.
• Providing wound care and other nursing interventions.
• Identifying changes in client wellbeing and recommending appropriate supports.
• Working closely with Care Partners, families and allied health professionals to achieve great client outcomes.
• Maintaining accurate clinical documentation and records.
• Supporting quality, compliance and safe service delivery.
• Helping clients remain independent and living well at home for longer.
