Job summary

My Community Health is seeking a Registered Nurse to join our team serving the NSW Central Coast. The role focuses on elderly and disabled clients, delivering high-quality home and community-based care on a part-time basis.

You will perform wound assessments, administer medications, develop care plans, monitor health, and collaborate with a multidisciplinary team to promote independence and dignity for our clients.

Qualifications

• Registered Nurse with current AHPRA registration.
• Minimum 2 years' experience in community nursing or similar role.
• Strong clinical skills in aged and disability care.
• Excellent communication and interpersonal skills.
• Ability to work autonomously and in a multidisciplinary team.
• Commitment to person-centred, compassionate care.
• Current driver's licence.

Responsibilities

• Provide nursing care and support to clients in their homes or communities.
• Conduct wound assessments and deliver simple or complex wound care.
• Complete holistic assessments and develop care plans.
• Administer medications and treatments as prescribed.
• Monitor client health and report changes to medical teams.
• Collaborate with healthcare professionals for coordinated care.

What you'll be doing

• Providing comprehensive nursing care and support to clients in their homes or community settings


• Conducting wound assessments and delivering both simple and complex wound care


• Completing holistic assessments, developing and implementing individualised care plans


• Administering medications and treatments as prescribed


• Monitoring client health and wellbeing, and reporting any changes to the appropriate medical professionals


• Collaborating with other healthcare professionals to ensure coordinated and holistic care


• Maintaining accurate and detailed client records

What we're looking for
