Healthcare Australia is seeking a Community Registered Nurse to join a dedicated team delivering person-centred nursing care to aged clients across the Adelaide community. Qualifikationen

• Current AHPRA registration as a Registered Nurse.
• Experience in aged care, community nursing or similar clinical environment.
• Strong clinical assessment and decision-making skills.
• Experience in wound management, medication administration and chronic disease management.
• Excellent communication and relationship-building skills.
• Ability to work autonomously while contributing to a multidisciplinary team.
• Genuine passion for supporting older people and delivering compassionate, person-centred care.
• Current driver’s licence and reliable vehicle.

Aufgaben

• Conducting comprehensive nursing assessments and developing individualised care plans.
• Providing clinical care and ongoing monitoring of aged clients in their homes.
• Medication management, wound care and chronic disease management.
• Monitoring changes in clients’ health and escalating clinical concerns appropriately.
• Supporting clients to maintain independence, wellbeing and quality of life.
• Liaising with GPs, allied health professionals, families and other healthcare providers.
• Maintaining accurate clinical documentation and care records.
• Providing clinical guidance and support to care staff where required.
