Job summary

Uniting is recruiting a Registered Nurse for Home Care in Western Sydney. You will deliver person-centred nursing care, assess needs, plan care, perform clinical interventions and monitor health in clients’ homes.

Work closely with families, carers and the care team to support independence, dignity and quality of life. We seek AHPRA-registered nurses with aged care/community nursing insight, strong communication skills, and a valid Australian driver licence with access to an insured vehicle.

Qualifications

• Current AHPRA registration and nursing qualification.
• Experience in aged care or community nursing.
• Commitment to person-centred clinical practice.
• Strong communication, organisation and time-management skills.
• Australian driver licence and access to an insured vehicle.

Responsibilities

• Provide high-quality, person-centred nursing care to clients at home across Western Sydney.
• Assess needs, develop and review care plans, and deliver clinical interventions.
• Monitor changes in health and respond to support safe, effective care at home.
• Coordinate care with clients, families, carers and the care team to promote independence and dignity.
