Job summary

Uniting Communities is seeking a Care Partner to join our Community Aged Care team in Adelaide. You will manage a caseload of clients receiving government-funded services, building relationships with clients, families and external providers to ensure care aligns with goals and standards.

Responsibilities include home visits, care plan updates, risk management and on-call duties, with emphasis on person-centred care and strong compliance.

Qualifications

• Relevant qualifications and/or demonstrated experience in aged care.
• Experience coordinating community-based support services.
• Knowledge of aged care programs and quality standards.
• Proficiency in Microsoft Office applications.

Responsibilities

• Manage and coordinate a caseload of clients receiving Support at Home services.
• Develop, review and update care plans to meet client goals and support needs.
• Conduct regular home visits, assessments and reviews.
• Monitor client wellbeing and respond promptly to changes in needs.
• Work collaboratively with families, representatives, clinical teams and external stakeholders.
• Identify, assess and manage client risks.
• Support quality service delivery through effective communication with rostering and support worker teams.
• Investigate and respond to client feedback and complaints.
• Complete accurate case notes, reports and compliance documentation.
• Participate in service improvement initiatives and quality assurance activities.
• Provide supervision and performance reviews for Support Workers.
• Participate in an after-hours on-call roster.
