Alwyndor in Adelaide is seeking a Support at Home Care Partner to help older people live independently at home. You will develop care plans, manage budgets and collaborate with clients, families and healthcare providers.

This role focuses on delivering high-quality home care, guiding team members and ensuring compliance with aged care standards. You will build strong relationships with communities while supporting continuous improvement.

Qualificações

• 2+ years of case management experience for older people.
• Knowledge of aged care legislation, funding, policy protocols and duty of care.
• Current driver’s licence and registered, roadworthy vehicle.

Responsabilidades

• Develop, maintain and update individualised care plans with clients, families and healthcare teams.
• Monitor service delivery and reassess care needs as required.
• Provide advice on service selection, self-management and future care options.
• Act as the primary contact for clients and families, addressing concerns and facilitating access to services.
• Manage budgets, monitor funding and meet productivity targets.
• Maintain accurate written and electronic records in line with legal and professional standards.
• Deliver services according to organisational policies, legislation, and Aged Care Standards.
• Support continuous improvement and ensure property and equipment are secure.
• Build and maintain positive relationships with consumers, families, service providers and government agencies.
• Promote a consumer-focused culture and resolve issues efficiently with empathy and transparency.
