Job summary

Helping Hand Aged Care in Salisbury South, South Australia, is seeking a dedicated Care Partner to support older people to live safely and with dignity at home. You will collaborate with customers, their informal supports, and internal teams to co-design personalised care plans and coordinate a range of services.

The role requires a Certificate III in Health Services Assistance, strong assessment and planning skills, a rights-based approach, a valid driver’s licence, and willingness to obtain

Qualifications

• Certificate III in Health Services Assistance or equivalent qualification.
• Experience in care management or case management with a rights-based approach.
• Exceptional assessment, support planning, and coordination skills.
• Sound understanding of person-centred and rights-based approaches.
• Commitment to professional development.
• Current driver’s licence and a reliable, insured vehicle.
• Senior First Aid and CPR certificates or willingness to obtain them.
• NDIS Worker Screening clearance or willingness to obtain one.

Responsibilities

• Provide rights-based, person-centred care to older people in their homes.
• Co-design tailored care plans and coordinate services with customers and supports.
• Monitor wellbeing and review support plans with customers regularly.
• Assist with budgeting and managing individual Support at Home budgets.
• Collaborate with internal teams and external supports to ensure coordinated care.
