Job summary

Uniting NSW & ACT is seeking a Neighbourhood Care Partner for our Home & Community Care team in Dubbo, NSW. The role involves case management, care planning and coordinating person-centred services to help clients live independently at home.

You will be the main contact for clients and networks, tailoring services to funding and preferences, and traveling to client homes and Uniting sites as required.

Qualifications

• Experience in aged care, especially Home and Community Care.
• Ability to manage client care budgets and allocate funds.
• Strong communication with clients, families and health professionals.
• Must hold a current Australian driver’s licence, own vehicle and comprehensive car insurance.
• Hold a current First Aid Certificate.

Responsibilities

• Provide person-centred case management and develop individual care plans.
• Coordinate services to help clients maintain independence.
• Advocate for client choice, dignity and informed decision-making.
• Manage client budgets to stay within funding.
• Build relationships with clients, families and service providers.
• Monitor and review care plans and client outcomes.
• Identify and escalate high-risk cases.
• Maintain accurate client records and documentation.
• Collaborate with Team Leaders and other internal teams.
• Respond to client feedback and complaints appropriately.
• Contribute to quality improvement and safe work practices.
• Travel between client homes, Uniting sites and community locations.
