Job summary

Life Without Barriers is seeking a Care Partner for Aged Care in the Moreton Bay area. This permanent full‑time role involves leading the delivery and continuous improvement of in‑home aged care services, managing client referrals, care plans, and coordinating home visits to ensure high‑quality, person‑centred support.

You will collaborate with families, internal teams, and external partners, review processes, and contribute to program growth while travelling across the region to visit clients

Qualifications

• Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for individuals and their carers, including people with culturally and linguistically diverse backgrounds.
• Experience in creating care plans, budgets, reports, correspondence and documentation relevant to programs.
• Confidence with technology including mobile devices and computers.
• Proficiency with Microsoft Office, Outlook and database systems.
• Ability to build relationships with diverse stakeholders.
• Ability to multi-task and work autonomously in a fast-paced environment.
• A current unrestricted Australian Drivers Licence, reliable vehicle and willingness to travel.

Responsibilities

• Undertake a comprehensive client assessment, risk assessment, client profile and care plan aligned to the relevant program.
• Conduct home visits and ensure care plans are monitored and reviewed on a regular basis in response to changing client needs.
• Ensure care plans are developed in partnership with the client/representative and other relevant stakeholders.
• Work collaboratively with internal teams, including Rostering and Practice Leads, to meet client needs.
• Develop strategies and/or broker services to address identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities.
• Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community.
