integratedliving Australia is hiring a Care Partner to support older Australians in Brisbane North, Sunshine Coast and surrounding communities. This role emphasizes relationship-building, personalised care planning and seamless in-home services.

You will coordinate supports, monitor care plans, and work with clients, families and providers to ensure safe, respectful and high-quality care in homes.

Qualifications

• Certificate IV or higher in Aged Care, Disability, Nursing, Community Services or a related field.
• Experience in care coordination, case management, aged care or community service delivery.
• Understanding of Support at Home services, care planning and client budgets.

Responsibilities

• Build meaningful relationships with clients, families and carers to understand what matters most.
• Create personalised care plans that promote independence and wellbeing.
• Coordinate services to ensure high-quality care in clients' homes.
• Welcome new clients into our services and settings.
• Conduct monthly monitoring calls to check wellbeing and changing needs.
• Manage client budgets and ensure services are delivered effectively.
