Job summary

integratedliving Australia is seeking a Care Partner to support clients across Brisbane North, Sunshine Coast and surrounding communities. You’ll coordinate care, build relationships with clients and families, and help maintain independence in home settings.

The role offers flexibility with remote work options, competitive salary (AUD 80,442.96 per year) plus 12% superannuation, travel allowance and salary packaging benefits, supporting older Australians in their communities.

Qualifications

• Certificate IV or higher in Aged Care, Disability, Nursing, Community Services or related field.
• Experience in care coordination, case management or community service delivery.
• Understanding of Support at Home services, care planning and client budgets.

Responsibilities

• Build meaningful relationships with clients, families and carers and understand their priorities.
• Create personalised care plans to promote independence and wellbeing.
• Coordinate services to ensure high-quality care in clients' homes.
