Job summary

Life Without Barriers is seeking a Care Partner to lead the delivery and continuous improvement of our Aged Care services across the Moreton Bay area. You will manage client referrals, support individuals as they commence services, and regularly review care plans to meet changing needs, coordinating home visits to ensure high-quality, person-centred care.

Qualifications

• Experience in all aspects of case management including assessment, planning, provision and evaluation of services.
• Experience creating care plans, budgets, reports and program documentation.
• Proficient with mobile devices and computers; comfortable using technology.
• Strong ability to build relationships with diverse stakeholders.

Responsibilities

• Undertake comprehensive client assessments, risk assessments, and care plan development.
• Conduct regular home visits and monitor and review care plans as clients' needs change.
• Develop care plans in partnership with clients and stakeholders.
• Collaborate with Rostering and Practice Leads to meet client needs.
• Develop strategies to address needs of Aboriginal and Torres Strait Islander clients in consultation with families.
• Represent Life Without Barriers at external forums and promote the organisation in the community.
