integratedliving Australia is seeking a Care Partner to support clients across Brisbane North, Sunshine Coast and surrounding communities. You’ll coordinate care, build relationships with clients and families, and help maintain independence in home settings.

Qualifications

• Certificate IV or higher in Aged Care, Disability, Nursing, Community Services or related field.
• Experience in care coordination, case management or community service delivery.
• Understanding of Support at Home services, care planning and client budgets.

What Does a Day as a Care Partner Look Like?

As a Care Partner, you'll be the trusted point of contact for your clients, helping them navigate their care journey and ensuring they receive the right support at the right time.

You'll be responsible for:

• Building meaningful relationshipswith clients, families and carers, taking the time to understand what matters most to them.
• Creating personalised care plansthat promote independence, wellbeing and quality of life.
• Coordinating services and supportsto ensure clients receive seamless, high-quality care in their homes.

What Will You Bring?

You’ll bring:

• ACertificate IV or higherin Aged Care, Disability, Nursing, Community Services or a related field, or relevant equivalent experience.
• Experience in care coordination, case management, aged care or community service delivery.
• An understanding of Support at Home services, care planning and client budgets.
• Strong communication skills and a natural ability to build trust and meaningful relationships.
• Excellent organisation and time management skills, with the confidence to work independently.
• A proactive, solutions-focused attitude and the ability to manage competing priorities.
• A current driver's licence and access to a reliable, comprehensively insured vehicle.
