Direct Care Australia on the Gold Coast is seeking an experienced Care Partner to join the Community Aged Care team. You will manage a caseload, conduct assessments, develop care plans and coordinate services to support older people to live safely and independently at home.

The role requires strong documentation, professional judgement, and the ability to travel across the Gold Coast, Brisbane and Sunshine Coast. A company vehicle is provided; this is a full-time, ongoing position.

Qualifikationen

• Experience in Home Care Packages, Support at Home or community aged care case management.
• Experience coordinating services and monitoring participant needs.

Aufgaben

• Care management: Independently manage a portfolio of Support at Home participants with regular engagement.
• Assessments and reviews: Conduct intakes, assessments and reviews including visits across Gold Coast, Brisbane and Sunshine Coast.
• Care planning: Develop person-centred care plans reflecting needs, goals and risks.
• Service coordination: Arrange and monitor services; coordinate nursing and allied health referrals.
• Budgets and funding: Monitor budgets and service utilisation to ensure funding is appropriate.
• Quality and documentation: Maintain case notes, care plans and records; escalate concerns.
• Participant engagement and growth: Build relationships with participants and families; respond to referrals and support service growth.
