Job summary

Integrated Living Aged Care is seeking a dedicated Care Partner to support clients across Brisbane North, Sunshine Coast and surrounding communities. You will coordinate care, build trusted relationships, and help clients stay independent in their own homes.

The role offers flexible work arrangements with travel to client sites. The ideal candidate will have a Certificate IV or higher in Aged Care or related field, strong communication skills and experience in care coordination.

Qualifications

• Experience in care coordination, case management or community service delivery.
• Strong communication and relationship-building skills.
• Ability to manage competing priorities and work independently.

Responsibilities

• Build meaningful relationships with clients, families and carers.
• Create personalised care plans to promote independence and wellbeing.
• Coordinate services to ensure high-quality in-home care.
• Welcome new clients into the services and monitor monthly wellbeing.
• Manage client budgets and ensure sustainable service delivery.
• Identify risks and resolve concerns with a proactive approach.
