The ideal candidate has at least 5 years of clinical nursing, with 2 years in case management, and strong EHR/telehealth skills for seamless remote collaboration.

Qualifications

• Current and unrestricted RN license.
• Minimum of 5 years of clinical nursing experience, with ≥2 years in case management.
• Strong knowledge of healthcare regulations, insurance processes, and community resources.
• Excellent assessment, planning, and problem-solving skills.
• Proficient with EHR systems and telehealth platforms.

Responsabilités

• Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.
• Coordinate patient care services, ensuring continuity and efficiency across different providers and facilities.
• Advocate for patients' needs, rights, and preferences throughout their healthcare journey.
• Liaise with insurance providers to obtain authorizations for services and manage coverage issues.
• Monitor patient progress, evaluate the effectiveness of care plans, and make necessary adjustments.
