Job summary

Charisma Healthcare Solutions is seeking a Senior Registered Nurse specializing in Case Management to lead remote patient care coordination and ensure optimal outcomes across settings.

You will work with patients, families and providers using EHR and telehealth tools, advocating for patient needs and coordinating services, with a focus on safe transitions of care and efficient resource use.

Qualifications

• Current and unrestricted Registered Nurse license.
• Minimum of 5 years of experience in clinical nursing, with at least 2 years in case management or care coordination.
• Strong understanding of healthcare regulations, insurance processes, and community resources.
• Excellent assessment, planning, and problem-solving skills.
• Proficient in using electronic health record (EHR) systems and telehealth platforms.
• Exceptional communication, negotiation, and interpersonal abilities for remote collaboration.

Responsibilities

• Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.
• Coordinate patient care services, ensuring continuity and efficiency across different providers and facilities.
• Advocate for patients' needs, rights, and preferences throughout their healthcare journey.
• Liaise with insurance providers to obtain authorizations for services and manage coverage issues.
• Monitor patient progress, evaluate the effectiveness of care plans, and make necessary adjustments.
• Educate patients and families about treatment options, available resources, and discharge planning.
