Job summary

Zohorecruit is seeking a Senior Registered Nurse – Case Management for a fully remote role. You will coordinate patient care, manage resources, and advocate for patients across settings, working with families, providers, and insurers to navigate the system and improve outcomes.

The ideal candidate has 5+ years of clinical nursing with at least 2 years in case management, an unrestricted RN license, familiarity with EHR and telehealth, and excellent remote collaboration and communication skills.

Qualifications

• Current and unrestricted Registered Nurse license.
• Minimum of 5 years clinical nursing experience, with at least 2 years in case management or care coordination.
• Strong understanding of healthcare regulations, insurance processes, and community resources.
• Excellent assessment, planning, and problem-solving skills.
• Proficient in using EHR systems and telehealth platforms.
• Exceptional communication, negotiation, and interpersonal abilities for remote collaboration.

Responsibilities

• Assess patient needs and develop comprehensive, individualized care plans.
• Coordinate patient care services across providers and facilities.
• Advocate for patients' rights and preferences throughout care.
• Liaise with insurance providers to obtain authorizations for services.
• Monitor progress, evaluate outcomes, and adjust plans as needed.
• Educate patients and families about treatment options, resources, and discharge planning.
