Job summary

Edge Executive Search Group is seeking a Senior Registered Nurse specialized in Case Management for a fully remote role. You will coordinate patient care, work with healthcare teams, insurers, and families to optimize outcomes and ensure smooth transitions of care across settings.

The ideal candidate has a valid RN license, at least 5 years of clinical nursing experience with 2 years in case management, strong communication, and proficiency with EHR and telehealth tools, in a remote capacity.

Qualifications

Current and unrestricted RN license.
Minimum 5 years clinical nursing with 2+ years in case management.
Knowledge of healthcare regulations and insurance processes.

Responsibilities

Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.
Coordinate patient care services across providers and facilities to ensure continuity and efficiency.
Advocate for patients' needs, rights, and preferences throughout their healthcare journey.
Liaise with insurance providers to obtain authorizations for services and manage coverage issues.
Monitor patient progress, evaluate care plan effectiveness, and adjust as needed.
Educate patients and families about treatment options, resources, and discharge planning.
