Job summary

Placements24, Vereeniging, Gauteng, ZA seeks a Senior Case Manager to lead high-level case coordination for individuals and families facing complex social challenges. You will develop, implement, and oversee individualized care plans and mentor junior staff while collaborating with external agencies.

You will drive program development, ensure accurate records, and advocate for clients, delivering compassionate, evidence-based support in a community-focused environment.

Qualifications

• Bachelor's degree in Social Work, Psychology, or Sociology.
• Master's degree is a plus.
• Minimum of 5 years in case management or social services.
• Experience supervising or mentoring staff.
• Strong knowledge of social services, local resources, and relevant legislation.

Responsibilities

• Manage a caseload of clients with complex needs and provide intensive case management.
• Develop, implement, and review individualized support plans with clients and stakeholders.
• Conduct comprehensive assessments to identify needs and barriers.
• Connect clients with community resources and programs.
• Monitor progress and adjust plans to achieve outcomes.
• Provide crisis intervention and client advocacy as needed.
• Maintain confidential client records and documentation.
• Collaborate with a multidisciplinary team for holistic care.
• Mentor and supervise junior case managers and support staff.
• Contribute to program development and evaluation; represent the organization at meetings.
• Engage with inter-agency partners in Vereeniging and surrounding areas.
