Formación

• Proven leadership experience in social care or community services.
• Ability to mentor junior staff and supervise teams.

Responsabilidades

• Overseeing and coordinating daily operations of community support programs.
• Developing, implementing, and evaluating individualised support plans for clients.
• Providing direct support and mentorship to clients, addressing their social, emotional, and practical needs.
• Supervising and training junior community support staff, ensuring adherence to best practices and organisational policies.
• Conducting risk assessments and developing safety plans for clients.
• Liaising with external agencies, healthcare providers, and families to ensure comprehensive care coordination.
• Maintaining accurate and confidential client records and documentation.
• Facilitating group activities and community engagement initiatives.
• Responding to emergencies and crisis situations with professionalism and calm.
