Placements24 is seeking a Senior Social Care Coordinator for a fully remote role supporting clients across multiple regions. You will oversee remote social care services, develop care plans, and ensure high standards of care through digital platforms.

The ideal candidate has extensive experience in social work or community care, exceptional organizational and communication skills, and a track record of mentoring staff while managing complex caseloads remotely.

Responsabilidades

• Lead and manage the coordination of remote social care services, ensuring effective and efficient delivery.
• Develop and implement comprehensive care plans, leveraging digital tools for client assessment and progress tracking.
• Provide remote guidance, support, and advocacy to clients facing various social challenges.
• Collaborate with community partners and external agencies via virtual communication channels to secure resources and services.
• Mentor and supervise junior social care staff, offering virtual training and support.
• Maintain meticulous and confidential digital records of client interactions, assessments, and outcomes.
• Stay abreast of current social welfare policies and best practices relevant to remote service delivery.
• Contribute to the design and refinement of remote program strategies and protocols.
