Qualifications

About the Role

Our client is a leading provider of comprehensive elderly care solutions and is seeking an experienced and dedicated Remote Case Manager to join their virtual team. This fully remote position allows you to provide essential support and coordination for elderly clients and their families from the convenience of your home office. You will be responsible for assessing care needs, developing personalised care plans, coordinating a range of services (including medical, social, and domestic support), and acting as a primary point of contact. This role demands a deep understanding of the challenges faced by the elderly and their caregivers, exceptional organisational skills, and a strong commitment to advocacy. Key Responsibilities

• Conduct comprehensive remote assessments of clients' physical, cognitive, social, and emotional needs.
• Develop, implement, and regularly review personalised care plans in collaboration with clients, families, and service providers.
• Coordinate a spectrum of services including healthcare appointments, medication management, domestic assistance, and social engagement activities.
• Act as the main liaison for clients and their families, providing regular updates, support, and addressing concerns promptly.
• Liaise with healthcare professionals, social workers, community resources, and external agencies to ensure seamless service delivery.
• Monitor client well-being and progress, adjusting care plans as needs evolve.
• Maintain accurate and confidential client records and documentation using our secure online case management system.
• Ensure adherence to all relevant regulations, ethical standards, and organisational policies governing elderly care and remote service provision.
• Identify potential risks and implement mitigation strategies to ensure client safety and well-being.
• Provide information and referrals to clients and families regarding available support services and benefits.

Requirements

• Bachelor's degree in Social Work, Gerontology, Nursing, or a related field.
• Minimum of 5 years of experience in case management, social work, or elder care, with a significant portion in a client-facing or coordination role.
• Demonstrated experience in care planning, needs assessment, and service coordination for the elderly.
• Strong understanding of the aging process, common geriatric conditions, and available support services.

• Proficiency in using digital communication tools, video conferencing platforms, and online case management software.
