Job summary

Placements24 is seeking a dedicated Social Care Facilitator in Polokwane to support the elderly in a home-based role. This part-time position involves direct care, companionship, and helping seniors access community resources to improve quality of life in Limpopo.

The ideal candidate has a caring disposition, strong communication skills, and readiness to coordinate with care managers, families, and healthcare professionals to ensure safe, dignified, and independent living for clients.

Qualifications

• Certificate or diploma in Caregiving or related field.
• At least 1 year experience with elderly or vulnerable adults.
• Valid driver's license and access to reliable transport.
• First Aid and CPR certification is a plus.

Responsibilities

• Provide direct personal care (hygiene, mobility, meals).
• Offer companionship and social activities for clients.
• Assist with appointments, errands, and transportation needs.
• Monitor well-being and report changes to care manager.
• Facilitate connections with community services and resources for seniors.
• Educate clients and families on available resources and care options.
• Maintain accurate and confidential client care records.
• Ensure a safe, comfortable home environment for clients.
• Coordinate with healthcare professionals and families to plan care.
• Promote client independence and dignity.
