Placements24 is seeking a compassionate Elderly Care Support Specialist in Upington, Northern Cape. The role combines home visits with office-based tasks, focusing on personalized care plans and advocacy for seniors.

You will assess needs, coordinate services, and provide emotional support while collaborating with families and healthcare providers to enhance independence and quality of life for elderly clients.

Qualifications

• Diploma or certificate in Gerontology, Social Work, Nursing, or related field.
• Minimum of 2 years of experience in elderly care, home care, or related support role.
• Strong understanding of the needs and challenges faced by seniors.
• Excellent communication, interpersonal, and listening skills.
• Patience, empathy, and a compassionate demeanor.
• Ability to work independently and manage time effectively.
• Proficiency in Microsoft Office Suite for record-keeping and communication.
• Knowledge of local community resources for seniors in the Upington area is a plus.
• A valid driver’s license and access to reliable transportation for home visits.
• Must be comfortable working a hybrid schedule, including in-home visits.

Responsibilities

• Conduct assessments of elderly clients' needs, including physical, social, and emotional well-being.
• Develop and implement individualized care plans in consultation with clients and their families.
• Coordinate services such as home care, medical appointments, and social activities.
• Provide emotional support and companionship to elderly clients.
• Assist clients with daily living activities as needed.
• Liaise with healthcare professionals, social workers, and other service providers.
• Educate families on resources and support systems available for elder care.
• Monitor clients' well-being and report any changes or concerns to supervisors.
• Maintain accurate and confidential client records and documentation.
• Organize and facilitate social events or group activities for seniors.
