Placements24 in Richards Bay, KwaZulu-Natal, is seeking a dedicated Disability Support Coordinator to join our expanding team. You will coordinate care plans, connect clients with essential resources, and advocate for their rights within the community.

The ideal candidate has a qualification in Social Work or a related field, experience in disability services, strong communication and organizational skills, and a valid driver's license to travel within the service area.

Qualifications

A relevant qualification in Social Work, Psychology, Community Development, or a related field.
Proven experience in disability support coordination or case management.
In-depth knowledge of disability types, legislation, and available support services.

Responsibilities

Assess the needs of individuals with disabilities and develop personalized support plans.
Coordinate and arrange necessary services, including healthcare, therapy, education, and social activities.
Liaise with families, caregivers, government agencies, and service providers to ensure seamless support.
Monitor the progress and well-being of clients, adjusting support plans as needed.
Provide information and guidance on available disability benefits and resources.
Advocate for the rights and inclusion of individuals with disabilities in the community.
Maintain accurate and confidential client records and documentation.
Organize support group meetings and community awareness events.
Ensure compliance with all relevant legislation and organizational policies in Richards Bay, KwaZulu-Natal.
Offer emotional support and encouragement to clients and their families.
