Qualifications

• Experience in disability services coordination or case management.
• Strong understanding of disability types and support needs.
• Excellent communication, negotiation, and interpersonal skills.
• Proficiency in assessment, care planning, and service coordination.
• Knowledge of resources and disability legislation in North West.
• Ability to work independently and manage a caseload.
• Empathy, patience, and client advocacy commitment.

Responsibilities

• Conduct comprehensive needs assessments for individuals with disabilities.
