Job summary

Qualifications

• A relevant qualification in Social Work, Occupational Therapy, Disability Studies, or related field.
• Minimum 2–3 years of experience in disability support or case management.
• Understanding of disability types, needs, and resources.
• Excellent communication and advocacy skills.
• Strong organizational and time-management abilities.
• Ability to work independently and within a team.
• Proficiency with computer systems for records and communication.
• Valid driver’s license and access to a vehicle for travel in George.
• Empathy, patience, and commitment to empowerment.
• Clear criminal record check.

Responsibilities

• Develop, implement, and review individualized support plans with clients and families.
• Coordinate access to services including healthcare, education, employment, and community programs.
• Provide direct support per the care plan.

• Maintain confidential client records and documentation.
