Job summary

Placements24 is seeking a compassionate Disability Support Specialist to join the Richards Bay team in KwaZulu-Natal. You will develop individualized care plans and provide direct support to promote independence for clients with disabilities.

Work with families and professionals to ensure coordinated care, advocate for clients' rights, and monitor progress. A patient, empathetic communicator with strong organizational skills will thrive in this role.

Qualifications

Relevant qualification in social work or related field.
At least 2 years' disability support experience.
Knowledge of disabilities and appropriate support strategies.
Understanding of disability rights and independent living.
Strong communication and interpersonal skills.
Ability to work with diverse backgrounds.
Experience in developing and implementing care plans.
Good organizational and record-keeping skills.
Patience, resilience, and a compassionate attitude.
Valid driver's license and ability to travel locally.

Responsibilities

Develop and implement individualized support plans.
Provide direct support with daily living activities and personal care.
Facilitate social inclusion and community participation.
Advocate for clients' rights with service providers and in the community.
Collaborate with families, caregivers, and healthcare professionals.
Monitor client progress and adjust plans as needed.
Educate clients and families about resources and services.
Assist clients in accessing employment, education, and recreation opportunities.
Maintain accurate and confidential client records.
Promote a safe, respectful, empowering environment.
