Qualifications

Responsibilities

• Develop and implement outreach strategies to connect with elderly individuals in the community.
• Identify needs and challenges faced by seniors, including social isolation, healthcare access, and daily living support.
• Organize social events, workshops, and activities to promote engagement and reduce isolation.
• Facilitate access to essential services such as healthcare, transportation, and social welfare programs.
• Build and maintain relationships with seniors, families, leaders, and partner organizations.
• Create and distribute informational materials about resources and services.
• Maintain records of outreach activities, client interactions, and program participation.
• Collaborate with the care team to ensure a comprehensive support network for seniors.
• Advocate for the needs of the elderly community to stakeholders.
