Qualifications

• A relevant qualification in Social Work, Gerontology, Nursing, or a related field.
• Minimum of 3 years of experience in elderly care, case management, or community support services.
• Demonstrated knowledge of aging challenges and needs.
• Familiarity with Benoni and Gauteng healthcare and social support services.
• Excellent communication, interpersonal, and active listening skills.
• Strong organizational skills and ability to manage multiple client cases.
• Proficiency in using virtual communication tools for remote coordination and reporting.
• Patience, empathy, and a respectful approach to working with older adults.
• A valid driver's license and access to reliable transportation for home visits.

Responsibilities

• Conduct assessments of the care needs and living situations of elderly individuals.
• Develop personalized care plans in consultation with clients, families, and healthcare professionals.
• Coordinate the delivery of services, including home help, medical appointments, and social activities.
• Liaise with medical practitioners, allied health professionals, and community services to ensure integrated care.
• Monitor the well-being of elderly clients, responding promptly to any concerns or emergencies.
• Provide emotional support and companionship to clients and their families.
• Assist clients with navigating healthcare systems and accessing available benefits and resources.
• Maintain accurate and confidential client records and progress notes.
