Job summary

Placements24 is seeking a compassionate Elderly Care Coordinator for Mossel Bay, Western Cape. The role combines in-person client visits with remote coordination, focusing on assessing needs, arranging services, and supporting families and healthcare partners.

A relevant qualification and 2+ years in elderly care are preferred. You will build care plans, liaise with providers, and advocate for seniors to live independently with dignity within a hybrid work model.

Qualifications

• Requires a relevant professional qualification in Social Work, Nursing, or Gerontology.
• Extensive experience in elderly care or case management strongly preferred.
• Strong communication, listening, and organizational abilities are essential.

Responsibilities

• Conduct comprehensive needs assessments for elderly clients, identifying physical, emotional, and social requirements.
• Develop personalized care plans in collaboration with clients, families, and healthcare professionals.
• Coordinate and arrange for essential services such as home care, medical appointments, transportation, and social activities.
• Liaise regularly with clients and their families to provide updates and address concerns.
• Build and maintain relationships with local service providers, healthcare facilities, and community resources.
• Monitor the quality of care provided and adjust care plans as needed.
• Maintain accurate and confidential client records and documentation.
• Provide emotional support and companionship to elderly clients.
• Organize and facilitate group activities and outings for seniors.
• Stay informed about available resources and support systems for the elderly in the Mossel Bay area.
