Key Responsibilities

• Conduct comprehensive needs assessments for individuals and families seeking support.
• Develop and implement individualized support plans tailored to client needs.
• Provide direct support, counseling, and guidance on a range of social issues including housing, employment, and health.
• Facilitate access to community resources and services, acting as a liaison between clients and external agencies.
• Organize and deliver community outreach programs and workshops to raise awareness and provide education.
• Maintain accurate and confidential client records and documentation.
• Collaborate with internal teams and external stakeholders to ensure integrated service delivery.
• Advocate for clients' rights and needs within the community and with relevant authorities.
• Monitor client progress and adjust support plans as necessary.

Requirements

• A relevant qualification in Social Work, Psychology, Community Development, or a related field.
• Proven experience (minimum 3 years) in a community support or social care role.
• Demonstrated understanding of social welfare systems and community resources in South Africa , particularly in the KwaZulu-Natal region.
• Excellent interpersonal and communication skills, with the ability to build rapport with diverse client groups.
• Strong assessment, planning, and problem-solving abilities.
• Proficiency in case management software and Microsoft Office Suite.
• Ability to work independently and as part of a team in a demanding environment.
• A valid driver's license and willingness to travel within the Pietermaritzburg area.
