Qualifications

• Experience in a community support or social care role.
• Understanding of social issues and community dynamics in South Africa.
• Excellent communication and active listening skills.
• Ability to work independently and in a team.
• Valid driver’s license and own transport advantageous for outreach activities.

Responsibilities

• Conduct comprehensive assessments of client needs, identifying areas for support and intervention.
• Develop and implement personalized care plans in collaboration with clients and relevant stakeholders.
• Provide emotional support, guidance, and practical assistance to clients facing life challenges.
• Facilitate access to community resources, including healthcare, education, employment, and housing services.
• Maintain accurate and confidential client records, documenting progress and outcomes.
• Organize and lead community outreach programs and workshops to raise awareness and provide education.
• Liaise with other professionals and agencies to ensure coordinated and holistic client care.
• Advocate for clients' rights and needs within the community and service systems.
• Respond to crisis situations with empathy and professionalism, implementing de-escalation techniques.
