Qualifications

• Proficient with case management software.
• Ability to work independently and in a team.

Responsibilities

• Conduct comprehensive needs assessments for individuals and families seeking support.
• Develop, implement, and monitor individualized care plans tailored to client needs and goals.
• Coordinate and facilitate access to a range of social, health, and educational services.
• Liaise with government agencies, NGOs, and other service providers to ensure integrated support.
• Provide advocacy and support to clients navigating complex systems and bureaucratic processes.
• Maintain accurate and up-to-date client records, case notes, and progress reports.
• Organize and deliver community outreach programs and workshops.
• Respond to client inquiries and concerns in a timely and empathetic manner.
