Placements24 seeks a diligent Case Manager to join the Port Elizabeth community, helping individuals and families access essential social services and supports. You will collaborate with local service providers, maintain confidential records, and monitor progress to ensure positive outcomes for clients in the region.

Qualifications

• Degree or diploma in Social Work, Community Services, or related field.
• Proven experience as a Case Manager or in a similar client support role.
• Strong knowledge of social welfare systems and community resources in the Eastern Cape.
• Ability to manage a caseload and work independently in the field.
• Excellent assessment, planning, and problem-solving skills.
• Effective communication and interpersonal abilities.
