Job summary

Placements24 is seeking a skilled Case Manager to join our social services team in Port Elizabeth. You will assess client needs, develop service plans, and coordinate support across housing, employment, health and social integration.

With a Bachelor’s degree and at least 2 years of experience, you will build client relationships, advocate for resources, maintain records, and contribute to positive community impact.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Minimum of 2 years of experience in case management or a related social service role.
• Knowledge of social welfare programs, community resources, and referral networks in Port Elizabeth.
• Excellent assessment, planning, and problem-solving skills.
• Strong organizational and time management abilities.
• Effective communication and interpersonal skills for building client relationships.

Responsibilities

• Conduct comprehensive assessments to identify client needs and eligibility for services.
• Develop individualized case plans in collaboration with clients, setting clear goals and objectives.
• Coordinate and link clients with appropriate community resources and support services.
• Monitor client progress regularly and adjust service plans as needed.
• Maintain detailed case notes, client records, and prepare timely progress reports.
• Advocate on behalf of clients to ensure they receive necessary support and resources.
