Placements24 is seeking an organized and dedicated Care Coordinator to join our client’s team in Welkom. You will coordinate services across healthcare providers and community resources, acting as the primary contact for clients and families.

The role requires nursing or social work qualification with at least 3 years’ experience in care coordination. Excellent communication and problem-solving skills are essential to advocate for clients and strengthen the Free State care network.

Qualifications

• Relevant qualification in nursing, social work, or a related health/community services field.
• At least 3 years of experience in care coordination, case management, or a similar role.
• Strong knowledge of local healthcare and social support systems in Welkom.
• Excellent communication, negotiation, and problem-solving skills.
• Proficiency in using client management software and standard office applications.
• Demonstrated ability to work independently and manage a caseload effectively.

Responsibilities
