Job summary

The Harris Center for Mental Health and IDD is seeking a Wraparound Facilitator to provide intensive case management through wraparound facilitation under supervision. You will assess, plan, and implement wraparound plans for youth and families, coordinating with schools, probation, and social service agencies to ensure trauma-informed, patient-centered care.

Qualifications

• Bachelor's Degree in a social, behavioral, or human service field.
• Prefer minimum of 1 year of experience in mental health.
• Must communicate with youth and their families to promote collaboration in wraparound plans.

Responsibilities

• Performs clinical assessments to determine service needs and progress toward consumer treatment goals.
• Formulates wraparound plans and secures LPHA signature authorization for consumers on caseload.
• Ensures services meet eligibility criteria and are authorized per agency guidelines.
• Provides skills training and psychosocial rehabilitation for severe mental illness.
• Intervenes in crisis to reduce symptoms and avoid admission to a more restrictive environment.
• Coordinates access to necessary care and services based on individual needs.
• Educates on the nature of mental illness and medications per Harris Center curricula.
• Monitors progress toward treatment goals and eligibility requirements.
• Contributes to suicide care and prevention within a trauma-informed framework.
