Harbor is recruiting a Care Coordinator to manage comprehensive, multi-system care for youth with complex needs and families. The role is full-time, remote, with local preference for Wood County residents.

Responsibilities include conducting assessments, linking community resources, facilitating Team meetings, and ensuring safety planning. You will work in home and community settings, possibly evenings or weekends, and maintain strong cross-system communication.

Qualifications

• At least 3 years of experience in providing community-based services to children/youth and families, or 2 years with an Associate or Bachelor degree, or 1 year with a Master degree.
• Background in family systems, community resources, case management, child counseling, or child development.
• Culturally competent with training to manage complex cases.
• Experience with youth with SED/trauma and inter-system engagement.
• Complete the high-fidelity wraparound training (COE-recognized).
• Clear verbal/written communication and representation in medical settings.
• Proficient computer use.
• Valid Driver’s license and insurable driving record.

Responsabilités

• Engages children/youth with behavioral health needs and their families for assigned caseload.
• Links youth and family to community resources and supports.
• Conducts CANS assessment and helps develop a strength-based care plan.
• Monitors and updates the plan to address challenges and barriers.
• Facilitates Child and Family Team meetings ensuring voice and choices.
• Encourages natural supports and community-based solutions.
• Coordinates clinical/psychiatric consultations for the youth.
• Develops safety/crisis plans and tracks CME-required activities.
• Willing to work flexible hours including evenings/weekends.
• Provides services in home and community settings.
