Everstand is seeking a Wraparound Facilitator to join our care team. You will engage high-need youth and families to develop, implement, and monitor intensive, individualized care plans, fostering a strong family partnership and ensuring fidelity to the Wraparound model.

The role requires coordinating meetings, resources, and safety plans within home and community settings. You will work with the care team to reassess needs, update plans, and support youth and families toward measurable outcomes
